BASEBALL SKILLS CAMP LIABILITY WAIVER AND RELEASE

Assumption of Risk: I understand that participating in a baseball camp involves physical activity and the risk of injury. I accept all risks.

Release of Liability: I release and hold harmless Bandits Baseball, Idaho Falls Chukars Baseball, Post 56 Baseball, the City of Idaho Falls, and all coaches, volunteers, and staff from any liability for injury, illness, or property damage related to the camp.

Medical Care: I authorize emergency medical care if needed and accept responsibility for any related costs.
